Training on the Integrated Community End-of-Life Care Support (ICEST) Services

Target participants

Health and social care professionals and supportive care workers, especially those who are working in the community elderly service units.

* Priorities will be given to those who have completed the basic level of https://foss.hku.hk/jcecc/online/learning or those who are working with patients and families facing end-of-life issues for more than two years.

Objectives

  • To promote the evidence-based and holistic community End-of-Life care
  • To enhance practitioners’ capacity in delivering ICEST model
  • To prepare for the upscaling of ICEST model in Hong Kong

Contents

  • Core principles of ICEST Model(Person-centered, evidence-based and needs-based)
  • The four stages of care planning delivery pathway, including identification, assessment, planning and intervention
  • 3Ps holistic need assessment and intervention framework
  • Skills practice (g. communication skills, assessment skills)
  • Worker’s emotional preparation in End-of-life care
  • Frontline practitioner sharing
  • Participants would be given
    • A certification of the training
    • Guidebook on ICEST model (Volume 1 for professional workers, Practice Guidebook for supportive care workers)
