The Project aims at providing community end-of-life care to life-threatening illness patients and their families, so as to support patients staying at home as their preferred place of care.

Project Objectives
- To strengthen the family relationship and rebuild the relationship with their loved ones
- To strengthen the family function to care the patients at home, including in-home support service and personal care service
- To facilitate elders dreams realized
- To bring memorable moments to the elders and families
Target Service Users
End-of-life patients with age 60 or above who live on Hong Kong Island, Shamshuipo, Kwai Tsing and Tsuen Wan are major target (Not included users in residential home)
Service Components
- Physical Care: eg. Symptom control, Carer education, Therapist assessment
- Psychosocial-spiritual Care: eg. User and carer’s emotion, family relationship, cheer up activities planning, life review, care plan, funeral plan, last wishes
- Practical Care: eg. Rehab aids lending, escorting and transportation, service referral
