This project aims to enhance the capacity of the Residential Care Homes for the Elderly (RCHEs) to be more willing, ready and able to provide quality palliative and end-of-life care in long term care settings. In the long run, the model of care could be widely adopted in all Government subvented Care-and-Attention Homes in Hong Kong with appropriate policy and funding.
Project Introduction
Project Objectives
- Development of a culturally congruent, locally applicable and evidence-based end-of-life care model for improving outcomes in care for the frail elders in RCHEs
- Increase the availability of a Standard Training Package for end-of-life care for professionals and frontline workers in aged care
- Successful reductions in the number of hospital admissions and avoidance of unnecessary futile interventions along the end-of-life care trajectories of frail residents in accordance to their expressed preferences
- Advance Care Planning as part of the care process in RCHEs
- Establishment of District Based Professional Support Team including its roles and responsibilities in sustaining end-of-life care services in RCHEs
- Raise the awareness and acceptance of the Hong Kong community towards advance care planning and advance directives for their roles in sustainable quality end-of-life care
- Promote advance directives and advance care planning to patients in RCHEs and their family members, and assist in the document handling for the patients in need
Target Service Users
- End Stage/Terminal ill residents in subvented RCHEs in Kowloon East, Kowloon Central and Kowloon West cluster (As of Nov 2023,69 subvented RCHEs have joined the Project to provide services. Please click here for details)
