“Life Rainbow” End-of-Life Care Services provides quality community end-of-life care to people with late stage chronic illness and their family members who live in Hong Kong East. It adopts collaborative approach and aims at fostering care partnership with hospitals, family members, patients’ self-help groups, professional and neighborhood volunteers to support families in need, so as to create a holistic community end-of-life care service model.

Project Objectives
- To enhance patients’ quality of life and enable them to live with dignity in the community as far as possible
- To empower family members’ capacity in performing their caring role and reduce their distress and sense of burden
- To help patients and their family members to think positively during the end of life
- To organize and empower community volunteers to support these families
Target Service Users
- End-of-life patients and their family members at home
- Serving patients with heart failure, chronic obstructive pulmonary disease, Parkinson’s disease, end-stage renal failure, severe stroke and advanced dementia
Service Components
Supported by registered social workers and nurses, to provide:
- Symptom management support
- Home care support
- Caregivers support on training on end-of-life care skill and stress management
- Personal and family counselling service
- Life review programme for end-of-life care patients
- Volunteer caring
