This project aims to enhance the capacity of the Residential Care Homes for the Elderly (RCHEs) to be more willing, ready and able to provide quality palliative and end-of-life care in long term care settings. In the long run, the model of care could be widely adopted in all Government subvented Care-and-Attention Homes in Hong Kong with appropriate policy and funding.

Project Objectives

- To provide end-of-life care at RCHEs in Kowloon Central district through capacity building of elderly homes and their staff, collaboration with Hospital Authority (“HA”), medical counterparts and community partners, and adoption of a protocol-based service pathway.
- To develop a culturally congruent, locally applicable and evidence-based end-of-life care model for improving outcomes in caring the terminally-ill elders in RCHEs
- To serve 16 subvented RCHEs for delivering end-of-life care services and test out the feasibility and effectiveness of implementing district-based service model
- To improve the quality of life and death of the frail residents in RCHEs and the quality of life of their relatives during the end-of-life journey
Target Service Users
- End Stage/Terminal ill residents in subvented RCHEs in Kowloon Central, their family members and staff of RCHEs (As of Nov 2023,69 subvented RCHEs have joined the Project to provide services. Please click here for details)
Service Components
- Capacity building of different levels of care providers (professionals and frontline) in participated RCHEs
- RCHEs implementation
- Collaboration with hospitals and Community Geriatric Assessment Service Team (CGATs)
- Promoting the concept of palliative care and advance directives
